Routine Home Care
Routine Home Care is the most common type of hospice care. It involves regular, scheduled visits from a dedicated, multidisciplinary care team who work to ensure the comfort and well-being of the patient. This team typically includes a hospice physician, registered nurse, hospice aide, social worker, chaplain, volunteer, and bereavement specialist. The care is provided in the comfort of the patient’s own home, allowing them to spend meaningful moments with loved ones while receiving expert medical and emotional support.
Continuous Care
Continuous Care is designed for periods of crisis when a patient requires intensive, around-the-clock nursing care to manage acute symptoms and maintain comfort at home. According to the Centers for Medicare and Medicaid Services (CMS) regulations:
“Continuous home care may only be provided during a period of crisis as necessary to maintain an individual at home. A period of crisis is a period in which a patient requires continuous care that is predominantly nursing care to achieve palliation or management of acute medical symptoms.”
This level of care is available in the patient’s home or a long-term care facility and is specifically reserved for times of acute need rather than for ongoing, routine support.
GIP (General Inpatient) Care
GIP care is provided in a hospice inpatient unit or facility when a patient’s symptoms cannot be adequately controlled in another setting, such as their home. This level of care focuses on intensive symptom management, particularly for those with short-term, complex medical needs that require expert medical attention. GIP care ensures that patients receive the specialized treatment and comfort they need in a safe and supportive environment.
Respite Care
Caring for a loved one with a terminal illness can be physically and emotionally demanding. Sometimes, caregivers need time to rest, recharge, or manage personal matters. Chicago Hope and Palliative Healthcare, Inc. offers short-term respite care in one of our inpatient facilities, subject to availability.
Respite care allows caregivers to step away, whether for a short vacation or personal time while ensuring their loved one continues to receive expert, compassionate care. Our highly trained hospice nurses and physicians provide round-the-clock support, prioritizing comfort and security.
We take a holistic approach to patient care, supporting both the patient and their family. If you are considering respite care, speak with your hospice team to make the appropriate arrangements. Please note that a maximum respite stay of five days is allowable.
